What is the item purchase suggestion form?

This form allows library users to suggest an eBook, electronic journal, or an item in another format for addition to the Virtual Library's collection.  The title and publisher field is required; all other fields are optional.  Please provide as much information as possible, as this will help us to  locate the requested item.
